Enrollee Materials

For State programs (Healthy Options, SCHIP, Basic Health Plus, Basic Health, Washington Health Program, Disability Lifeline) Community Health Plan makes available enrollee benefit materials and posts information on the web site describing plan services and features. The handbooks, other printed materials, and web content contain helpful information about how to use the plan, its benefits, enrollee rights and responsibilities, and more.

In the new member packet, Healthy Options, CHIP, and Basic Health Plus enrollees get:

  • A printed Healthy Options Supplement, which contains important benefit and service information, including excerpts from the State-published all-Plan benefit handbook that will be sent to enrollees sometime in 2011.
  • Additional up-to-date information such as enrollee privacy and other rights; grievances and appeals; and utilization management practices and policies.
  • A listing of information enrollees can find on the Community Health Plan web site.

In the new member packet, Basic Health enrollees get:

  • A postcard the enrollee can return to request a free, printed Basic Health Medical Benefits Summary.
  • A listing of information enrollees can find on the Community Health Plan web site. This information can be downloaded from the web site or will be sent in printed form to anyone who requests it.

In the new member packet, Disability Lifeline enrollees get:

  • Two ways to request a printed Disability Lifeline Medical Benefits Summary: call Customer Service or pick up a book at their CSO or clinic.
  • A listing of information enrollees can find on the Community Health Plan web site. This information can be downloaded from the web site or will be sent in printed form to anyone who requests it.
